What is Remote Patient Monitoring (RPM)?

What is required?
Data must be reviewed by a registered nurse (RN), nurse practitioner (NP), clinical nurse specialist (CNS) or physician assistant (PA), who is responsible for reporting data to the prescribing physician in the event of a measurement outside the established parameters.

Our platform configures with to 97% of wearable devices currently on the market which allows a patient to monitor their own blood pressure, blood glucose measurements, and heart rate.
Our platform is able to collect a patient’s vital statistics and transport it back through our secure cloud-based platform. It is then used to prioritize patient care based on his or her monitoring parameters set by their clinician.
Patient readings that are out of range are flagged and alerts are sent to the patient and their identified care team.
