What is Remote Patient Monitoring (RPM)?

Remote Patient Monitoring is the ability to monitor patients beyond the doors of the clinic. By providing connected devices to patients, we are providing the tools to succeed at managing the chronic conditions in the home with the comfort of knowing their clinician is monitoring for any issues before they become emergencies. This allows clinicians the ability to deliver quality care to a larger number of patients and tap into additional revenue sources through Medicare.

What is required?

Data must be reviewed by a registered nurse (RN), nurse practitioner (NP), clinical nurse specialist (CNS) or physician assistant (PA), who is responsible for reporting data to the prescribing physician in the event of a measurement outside the established parameters.

Our platform configures with to 97% of wearable devices currently on the market which allows a patient to monitor their own blood pressure, blood glucose measurements, and heart rate.

Our platform is able to collect a patient’s vital statistics and transport it back through our secure cloud-based platform. It is then used to prioritize patient care based on his or her monitoring parameters set by their clinician.
